Announcement (Effective August 29th, 2023)

Notice to All Recent Nurse Aide Training Graduates
You must submit a completed Initial Application FORM 283B directly to CDPH by mail or Portal to be added to the Nurse Aide Registry. Please do not wait until after you’ve passed your exams to submit these forms for processing. Submit these forms upon Training Program completion. Please see resources below for more information.

283B (and 932) Forms

  1. When Applying to Test:

Candidates are required to upload their 283B forms (or 932 forms) into their CNA365 accounts so Credentia staff can verify training completions and state authorization to test. This verification process could take up to 2 to 3 business. Additionally, if you do not have a driver's license or state ID number, please write "N/A" on that section of the form before uploading.

Exam Start Times

The test start times in CNA365 represent the time that the exam day is scheduled to begin and NOT when the exam type will begin. 

Example: A candidate who would like to test on January 30, 2023, can select the 8 am Written exam AND the 8 am Skills exam for the same day and location in CNA365’s Exam Scheduling section. The test times will not overlap because 8 am represents the overall time the test day begins. Candidates are still advised to arrive at least 30 minutes before the scheduled exam day start time for check-in.

Announcement (Effective April 25th, 2023)

Candidates that have not been added to the registry within sixty (60) days after passing both portions of the exam should contact the CDPH CA office at (916) 327-2445 for the status of their registry inclusion. Please follow the automated prompts and then select “0” to speak with an agent. The office hours are Monday through Friday, 8 am – 5 pm PST.

COVID-19 Policy Reminder!

 Per the National Council of State Board of Nursing, we are still implementing the COVID-19 Testing Requirements for candidates. This includes supplying and wearing your own face mask for the exam.  If you do not bring a face mask to testing, you will not be permitted to test. Please see the full COVID-19 testing requirements in the Resources section, under the COVID-19 heading.

Steps to Register for the NNAAP Nurse Aide Exam

Complete Nurse Aide Training Program

Student must complete the state-approved nurse aide training program.

If you did not complete a state-approved training, you may still register under a different eligibility route as listed in the Candidate Handbook.

Review Candidate Handbook

Your Candidate Handbook is your handy guide to answer all the questions you have about submitting an application, scheduling, taking an exam, getting score results, and much more.

Create Your CNA365 Account

Create an account with our online CNA365 system to register for testing by clicking the "Sign Up" link on the login page. You will need to submit an application for testing in order to register for exams.

We highly recommend that you read the Registration Quick Reference Guide with detailed instructions on how to register online for your exam BEFORE you begin the registration process.

Did you previously test through the Northern Regional Testing Center?

Attention Candidates who have previously tested under the NRTC!

If you have previously tested the National Nurse Aide Assessment Exam through the Northern Regional Testing Center (NRTC), when you create a CNA365 account, all of the information from your previous NNAAP exams will carry over to your Credentia CNA365 account.

Northern California Examination and Scheduling Services

Create or Log into your CNA365 account to:

  • Change your current address and phone number.

  • Submit Correction Form to update your name, date of birth, or social security number before your scheduled exam.

  • Obtain and submit an Examination Testing Application.

  • Check Testing Application status.

  • Find test sites and availability.

  • Schedule, reschedule or cancel an examination.

  • View exam details such as examination type, scheduled time, date, and location.

  • View the Score report.

  • Submit and review a Grievance application for completed examination.

  • Submit an excused absence application.

Contact Customer Service when:

  • Problem occurs when creating a CNA365 Account.

  • Did not receive an activation link.

  • Problem occurs when applying an assigned voucher code.

  • Testing Application Issue.

  • Received an error message when scheduling an examination.

  • Score report is not viewable in CNA365 after 24 hours from your exam.

California of Public Heath (CDPH)

Contact the California of Public Health (CDPH) to:

  • Obtain information regarding requirements for initial licensing

  • Change your current address or name after testing (please submit a Demographic application in CNA365 to reflect the changes as well)

  • Obtain information regarding reciprocity (these applications are not processed through CNA365)

  • Obtain information regarding renewing an existing certification (these applications are not processed through CNA365)

  • Obtain additional registry information and assistance. If you’ve passed both exams and submitted all required documentation on the CDPH Healthcare Document Upload Portal and it’s been over 30 days, please contact CDPH at CNA@CDPH.CA.GOV and include your name, DOB, mailing address, last 4 digits of your SSN, and specify the dates in which you passed your exams with Credentia.
